State laws require laboratories such as Quest Diagnostics to obtain confirmation from the ordering provider that informed consent has been received from their patients for certain genetic tests if the testing is performed in Massachusetts, or if the ordering provider is located in one of these states: AK, AZ, DE, FL, GA, IA, MA, MN, MT, NV, NH, NJ, NY, OR, SC, SD, or VT.
A blanket confirmation of informed consent confirms compliance with the applicable informed consent laws and ensures that any genetic tests ordered will not be delayed.

If you prefer to provide confirmation of informed consent with each applicable genetic test ordered, you may do so by adding test code 1315, “Confirmation of Patient Informed Consent for Genetic Testing,” to applicable orders. An individual patient confirmation of informed consent form can also be found at QuestDiagnostics.com/consent if paper is preferred.
