MEDICAL AND DENTAL INSURANCEAthena Diagnostics wants its employees and their families to have good medical care and dental benefits at affordable rates. Medical coverage begins after 30 days of service. Dental coverage begins after 90 days of service. VISION INSURANCEWith the EyeMed Access Plan, Athena Diagnostics offers a comprehensive plan with a well-established network that is easy to use. Vision coverage begins after 90 days of service, following the employee's date of hire. FLEXIBLE SPENDING ACCOUNTThe Flexible Spending Account Plan allows employees to pay for up to $5,000 unreimbursed medical/dental expenses and $5,000 in dependent care expenses each year with pre-tax dollars through payroll deductions per year. 401(K) SAVINGS AND INVESTMENT PLANEvery calendar quarter, the Company will contribute 4% of the employee’s quarterly earnings into their 401(k) accounts, regardless if the employee participates in the plan. Participation in this plan is voluntary and employees are encouraged to participate immediately upon hire. GROUP LIFE ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) INSURANCEAthena Diagnostics, Inc. pays 100% of the premiums for employee life insurance and accidental death and dismemberment insurance. Coverage begins after 90 days of service. SHORT-TERM AND LONG-TERM DISABILITY PLANSAthena Diagnostics, Inc. pays 100% of premiums for these plans. These plans provide benefits to employees if they become ill or disabled and by doctor’s orders are unable to work. Coverage begins after 90 days of service. PTOIn addition to the company’s regularly scheduled holidays, Athena Diagnostics, Inc. provides vacation, floating holidays, sick time, and personal time for employees to celebrate important occasions, relax, and attend to personal matters. EMPLOYEE ASSISTANCE PROGRAM The Employee Assistance Plan is free and can be used as soon as you are hired. It is a confidential program that provides counseling and referral services to help employees and their families resolve personal or work issues that may be affecting work, home, and family life. Coverage begins on the employee’s date of hire. TUITION ASSISTANCE PROGRAMAthena Diagnostics will reimburse employees 100% of tuition and related costs, (i.e., registration fees, lab fees, and textbooks) up to a calendar year limit, for courses taken towards a degree either directly related to their job, or courses to prepare them for greater responsibility within our business environment. To be eligible, employees must have one year of continuous employment, a satisfactory work performance level, and management approval prior to course registration. Part-time employees can look forward to a wide variety of benefits too! Summaries of these benefits are listed below. PTOAthena Diagnostics provides employees scheduled to work 30 hours weeks, eight days of vacation per year. Employees accrue vacation for each full week of work. Employees who are scheduled to work less than 30 hours per week are not eligible. PAID HOLIDAYSAthena Diagnostics currently provides eight regularly scheduled holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and the day after, Christmas Eve and Christmas Day. Employees who are scheduled to work 30 or more hours per week will be paid six hours for each holiday.
